Bio-stimulation is understood to be the optimization of the physiological state of the skin, for a real improvement of its functions.
In the dermis, the most important cell is fibroblast: which is able to guarantee, on the basis of its physiological conditions (age and state of the matrix), the continuous production of various proteoglycans, particularly of the hyaluronic acid, elastin and different types of collagen.
The physiological stimulation of the fibroblasts is conditioned by the degree of pH of the surrounding environment and the inputs that arrive at particular receptors positioned on their surface.
The hyaluronic acid fragments, particularly those between 20 and 38 monomers, activate the CD44 receptors (surface receptors) and become similar, as a result, to PDGF (Platelet-Derived Growth Factor). Their activation induces, in particular, the formation of reticular collagen. On the contrary, the stimulation of CD39 and CD40 receptors leads to the formation of fibrotic collagen.
Until now, bio-stimulation with hyaluronic acid was performed by injecting non-cross-linked products with an average molecular weight of 1,000,000 Da and with a concentration ranging from 15 to 20 mg/ml.
Even if the obtainable results with these parameters are still considered as satisfactory, in-depth researches conducted in our labs in recent years, validated by numerous in vivo trials, and confirmed in a large international bibliography, have shown that the biological responses elicited from hyaluronic acid are strictly dependent on its molecular weight.
In fact, the accumulation of hyaluronic acid with low Molecular Weight represents for the fibroblast a precocious signal of alteration of the ECM (extracellular matrix), while the preponderance of hyaluronic acid with high Molecular Weight is an indication of good homeostasis: the most intense bio-stimulation will be obtainable, therefore, with fractions of hyaluronic acid with high Molecular Weight, greater than 1,000,000 Da.
